Trinity will provide you with your newborn's initial birth certificate. If you want to obtain one at a later date, you must go to the appropriate governmental agency or you can simply log onto www.dshs.state.tx.us/vs/default.shtm at the Texas Department of State Health Services.

How do I request a Social Security number for my newborn?
Parents may request a Social Security number for their newborn at the time of birth. The Social Security Administration will assign your newborn a number and mail the card directly to you.
