FOR PHYSICIANS / NURSES
1. Do you accept applications for internships and residencies?
Trinity is not a teaching facility and we do not have these opportunities for physicians, but we can accommodate some RN nursing internships. Please contact the HR recruiter at (972) 394-2281, or via e-mail at Diana.reeves@hospitalpartners.com

2. I'm a physician interested in relocating to your area - who can I speak with about opportunities at your facility?
Trinity recruits physicians from various specialties depending on the community need at the time. Please e-mail a copy of your current CV to Triphysicianrecrui@hospitalpartners.com. If there is a need for your particular specialty at Trinity Medical Center, you will be contacted. If you are interested in applying for privileges at Trinity, contact the Medical Staff Services office at 972-394-2254.

3. Do you have any sort of internet site for physicians where I can access reports?
Yes, Trinity offers physicians access to the "Physician Portal" which allows you to pull certain reports and admitting information directly from any computer with an internet connection. You must contact the Information Systems department at (972) 394-2409, or Paul Ratcliff (Director) at (972) 394-2366 in order to be set up for access.

4. How can I be listed on the Physician Referral page or update my information?
Active staff physicians may opt into this referral program by completing a profile. Simply contact the physician relations department @ 972-394-2381 or email Susan.watson@hospitalpartners.com to obtain a copy.

FOR THE GENERAL PUBLIC
1. How do I obtain a copy of a birth certificate for someone born at Trinity Medical Center?
Birth Certificates are completed by the hospital and sent to Texas Vital Statistics within five days of birth. Please contact the Denton County Clerk's Office directly at Metro (940) 349-2010 or (972) 434-8820 (Birth & Death Certificates). For more information visit their website at www.dentoncounty.com or you may directly contact the Bureau of Vital Statistics at 512-458-7111 or visit their website at http://www.texasonline.com.

2. How do I request a Social Security number for my newborn?
Parents may request a Social Security number for their newborn at the time of birth. The Social Security Administration will assign your newborn a number and mail the card directly to you.

3. How can I get my medical records from your facility?
Please call Medical Records at 972-394-2492 or via the Release of Information Request line at 972-394-2272. You will need to complete and sign an authorization for release of medical records/patient information and return it to the Medical Records department before they will release your records. You may download this form, complete, and fax to: 972-391-2351.

4. Who should I speak with about my hospital bill?
Please contact our Regional Business Office directly at 866-289-6943 for all hospital billing inquiries. They are located offsite and do not have an office within the hospital. (*Please note that certain physician groups, such as radiology, pathology and emergency physicians are not part of the hospital and perform their own billing.) If your inquiry pertains to services rendered before September 1, 2007, call 214-387-6400 or 800-346-0775.

5. I'll be giving birth at your facility. How many people may I have in the room with me?
Two is the maximum number of people who may be with you at one time during the delivery.

6. My friend or family member recently had a baby. Can I see their baby's photo online? Click here

7. I can't be there for my family member's surgery, Can I get a message to them?
Yes, please click here to try our E-mail a Patient function. Your message will be delivered usually within the same day or next business day, although on some occasions delivery may not be possible if the patient has been discharged before receipt of e-mail.

8. I'd like to take a CPR class. Does your hospital offer these to the public?
Unfortunately we only provide CPR training to our staff. Your Dallas American Heart Association provides certification classes. Contact them at 877-242-4277 or 877-AHA-4CPR or by clicking here to visit their website CPR page to find a class near you.

9. What is the closest train station or bus stop to Trinity?
The closest DART bus stop is on Route 331, which stops right near the hospital. This is in quadrant A of the transit map. For more information, please visit the DART website at www.dart.org or their maps page at http://www.dart.org/maps.asp.

10. What are your policies regarding visitors for patients? Standard visiting hours are 9am - 9pm. Please note that the main doors lock at 9pm for security reasons, and all admittance to the facility after 9pm is via the Emergency Department on the northwest side of the hospital. 11. When is your cafeteria open for hospital visitors?
Breakfast is available weekdays from 7am - 9am, and lunch is served from 11:00am to 1:30pm Monday through Friday. The cafeteria line is closed for dinner and on weekends, but there are vending machines and coffee available 24 hours a day. Vending machines are also located in the hallway from the main hospital to the plaza offices.
